Supervisor - PACE Program Adult Day Health Center
About the Role
Under the supervision of the Operations Manager, the Supervisor manages the daily operations of the PACE Center. Responsibilities include the planning, coordination, and implementation of all operations in the PACE Center. Works in conjunction with Social Workers to assure concerns and support are followed up with families, participants, and caregivers. Collaborates with the Therapy team (PT/OT/ST/RT) to ensure recreational activities and wellness classes are provided as scheduled. Works with the Registered Dietician to deliver an excellent dining experience and ensure diets are provided as ordered. Works with the Triage RN to ensure Program Aides complete assigned daily tasks and report areas of concern. At the Alternative Care Site, the Supervisor serves as the Health Care Coordinator for ADHS.
Responsibilities
- Manage daily operations of the PACE Center, including planning, coordination, and implementation.
- Collaborate with Social Workers to address concerns and support for families, participants, and caregivers.
- Work with the Therapy team (PT/OT/ST/RT) to ensure scheduled recreational activities and wellness classes.
- Coordinate with the Registered Dietician to deliver an excellent dining experience and ensure proper diets.
- Ensure Program Aides complete assigned daily tasks and report areas of concern in conjunction with the Triage RN.
- Serve as the Health Care Coordinator for ADHS at the Alternative Care Site.
- Handle potentially crisis/emergency situations appropriately and effectively.
- Maintain appropriate participant-to-staff ratios and adapt to necessary staffing situations.
- Participate actively in the team process and direct issues or concerns about the program to the supervisor.
- Maximize productivity while maintaining high quality.
Requirements
- Associate's Degree in Nursing (Required).
- Bachelor’s in Nursing or other healthcare-related field (Preferred).
- Registered Nurse licensure (Required).
- NC Driver’s Licensure (Required).
- BLS and First Aid certification (Required).
- Five (5) years of related experience, with at least one (1) year working with a frail or elderly population (Required).
- Two (2) years of related experience with at least one (1) year working with a frail or elderly population (Preferred).
Skills
- Experience working effectively within an interdisciplinary team model.
- Ability to plan, prioritize, and follow up on/delegate responsibilities effectively.
- Working knowledge of physical, mental, spiritual, transportation, and social needs of the frail/elderly and their families.
- Demonstrated effective leadership and decision-making skills with at least 1-year experience.
- Previous supervisory skills, including coaching and mentoring.
- Knowledge of principles, practices, standards, and techniques of personal assistance services.
- Working knowledge of quality improvement and cost containment systems.
- Confidence and positivity in an environment with impaired adults.
