Expected pay for this position is $25.10 - $29.54/hour. Actual pay will be determined by experience, skills, and internal equity.
Overview
This position is located in Peoria, IL, with a schedule of Monday–Friday, 8:00 AM – 4:30 PM.
Position Summary
The Supervisor of Digital Health will collaborate with OSF OnCall clinical and nonclinical teams to enhance access to healthcare and improve the health of vulnerable residents as they care for themselves in home and community settings. Responsibilities include overseeing the day-to-day operations and activities of Digital Health Workers (DHWs) and Digital Health Navigators (DHNs) within their hub. The role serves as the main contact for senior leaders and frontline operations, ensuring alignment between community initiatives and organizational goals.

Required Qualifications
- Associate's Degree
- 1 year of experience in a service-related position
- Community Health Worker certification within 1 year of hire
- Excellent interpersonal and communication skills
- Proficiency with Microsoft software and solid computer skills
- Strong analytical and problem-solving skills with attention to detail
- Ability to maintain professionalism with physicians, clinicians, community organizations, and other professionals
- Proficiency in English for community interactions (in person, by telephone, and digitally)
- Demonstrated ability to work independently

- Bachelor's Degree
- 2 years of experience in healthcare operations and/or experience with social work or community health work
