Speech Therapist (SLP), PRN

About the role

Chandler Creek Post Acute is a 133-bed skilled nursing facility in Greer, South Carolina. We are seeking a Speech-Language Pathologist (SLP) to join our rehab team on a PRN basis.

Key Responsibilities

• Evaluate residents within 24 hours (or 1 business day) of physician referral
• Develop and implement individualized treatment plans with physician approval
• Provide therapeutic interventions based on patient needs and goals
• Maintain accurate and timely clinical documentation
• Participate in care conferences, discharge planning, and interdisciplinary meetings
• Educate staff and caregivers on therapy techniques and maintenance programs
• Supervise Speech-Language Pathologist Assistants (SLPAs) as applicable
• Conduct in-service training for facility staff

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology
• Active, unencumbered SLP license in South Carolina (required)
• Certificate of Clinical Competence (CCC-SLP) preferred
• Experience in a skilled nursing or rehab setting preferred
• Strong knowledge of speech, language, cognitive, and swallowing disorders
• Familiarity with PointClickCare (PCC) and Casamba preferred
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
