About the Role

The Speech-Language Pathologist evaluates and treats patients with speech, language, cognitive, voice, and dysphagia deficits in a home setting. They develop and implement individualized treatment plans, perform assessments, and modify interventions based on ongoing evaluations.

Responsibilities

  • Perform diagnostic assessments and interpret results to determine patient needs.
  • Develop, implement, and adjust personalized treatment programs tailored to patients’ age and capabilities.
  • Deliver therapy using appropriate strategies and activities to promote health and achieve goals.
  • Coordinate care plans with medical, nursing, and rehabilitation teams.
  • Maintain thorough documentation, including evaluations, treatment plans, and progress notes.
  • Communicate effectively with patients, families, and team members about progress and needs.
  • Participate in departmental program development and supervise clinical interns.

Qualifications & Skills

  • Current CCC-SLP certification from ASHA or active supervised clinical fellowship, with certification within one year of employment, or active state licensure.
  • Preferred 1-3 years of experience in diverse healthcare settings.
  • Strong communication, teamwork, and documentation skills.
  • Ability to work effectively with patients of all ages, demonstrating compassion and professionalism.
