About the Role
The Speech-Language Pathologist evaluates and treats patients with speech, language, cognition, voice, and dysphagia deficits in a home setting. They develop individualized treatment plans, perform assessments, and deliver direct therapy using age-appropriate strategies. The role involves collaborating with interdisciplinary teams, documenting progress, and participating in program development and student internships.
Responsibilities
- Conduct assessments and interpret diagnostic tests to determine patient needs.
- Create and adjust personalized treatment programs based on ongoing evaluations.
- Provide therapy tailored to patients’ physical, emotional, and intellectual capabilities.
- Maintain detailed documentation, including evaluation results and progress notes.
- Communicate effectively with patients, families, and healthcare teams.
- Participate in departmental program development and supervise clinical internships.
Requirements
- Current CCC-SLP certification or active state licensure.
- Preferred 1-3 years of experience in diverse settings such as hospitals, SNFs, outpatient, or home health.
- Strong communication, teamwork, and confidentiality skills.
- Ability to perform job functions with adequate visual acuity and hearing.
