About the Role

The Speech-Language Pathologist evaluates and treats patients with speech, language, cognition, voice, and dysphagia deficits in a home setting. They develop individualized treatment plans, perform assessments, and deliver direct therapy using age-appropriate strategies. The role involves collaborating with interdisciplinary teams, documenting progress, and participating in program development and student internships.

Responsibilities

  • Conduct assessments and interpret diagnostic tests to determine patient needs.
  • Create and adjust personalized treatment programs based on ongoing evaluations.
  • Provide therapy tailored to patients’ physical, emotional, and intellectual capabilities.
  • Maintain detailed documentation, including evaluation results and progress notes.
  • Communicate effectively with patients, families, and healthcare teams.
  • Participate in departmental program development and supervise clinical internships.

Requirements

  • Current CCC-SLP certification or active state licensure.
  • Preferred 1-3 years of experience in diverse settings such as hospitals, SNFs, outpatient, or home health.
  • Strong communication, teamwork, and confidentiality skills.
  • Ability to perform job functions with adequate visual acuity and hearing.
