About the role

The Skilled Rehab and Home Health team at Good Samaritan Grand Island and Hastings seeks to hire a Physical Therapist to join our team. The position is full time, covering the Skilled Nursing Facilities and surrounding area Home Health clients. New graduates are welcomed. Paid travel and mileage between home health patients.

This role administers speech and language evaluations, tests, or examinations to diagnose and plan treatments for clients with various speech problems and swallowing disorders. You will assess clients with speech, language, cognitive, or swallowing problems and develop individualized treatment plans based on clinical diagnoses. The position involves providing speech and language services, monitoring disease progression, and adjusting treatments accordingly. You will instruct clients in practicing therapeutic exercises, ensure compliance with standards and policies, and demonstrate documentation in accordance with departmental guidelines.

You may treat individuals of all ages with varying diagnoses and disorders and will interact with providers, case managers, third-party payers, and public organizations. Supervision of assistants, aides, and students may be required, along with providing ongoing education and training to peers. Work may focus on post-acute, long-term care, and home health care environments.

Responsibilities

  • Administer speech and language evaluations, tests, or examinations to diagnose and plan treatments for clients with speech problems and swallowing disorders.
  • Assess clients with speech, language, cognitive, or swallowing problems and develop individualized treatment plans based on clinical diagnoses.
  • Provide speech and language services to clients; monitor disease progression and adjust treatments accordingly.
  • Instruct clients in practicing speech and language therapeutic exercises; provide consultation for clients working through treatment processes.
  • Ensure all speech and language-related standards, policies, protocols, and quality requirements are met.
  • Demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements.
  • Follow and adhere to accepted clinical practice guidelines of professional organizations.
  • Exhibit flexibility with acceptance of variable work schedules/assignments.
  • Participate in activities to promote the department, Good Samaritan Society, and Sanford Health.
  • Interact with individuals outside of the department, including providers, case managers, third-party payers, and public organizations.
  • Supervise assistants, aides, and students as they provide client care, treatment, or other services.
  • Provide ongoing education and training to peers in areas of expertise as determined by department leadership.
  • Perform other related duties as requested by manager.

Requirements

  • Appropriate education level required in accordance with state licensure.
  • Must be licensed in the state(s) of practice as a Speech-Language Pathologist.
  • When working at a Sanford Health Facility (preferred by GSS Facilities): Holds a Certificate of Clinical Competence issued by the American Speech-Language Hearing Association (ASHA) or currently working on Clinical Fellowship Year (CFY).
  • When applicable, may require a valid driver’s license.
  • Obtains and subsequently maintains required department-specific competencies and certifications.
  • Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.

Schedule

  • Full time, 35-40 hours per week.
  • Weekly hours: 36.00.
  • Shift: Day (weekdays, weekends, and holidays can be flexible).

Pay

Salary Range: $32.50 - $52.00 per hour.
