Speech Language Pathologist (SLP) - PRN
About the role
The Speech-Language Pathologist (SLP) at Concord Post-Acute will evaluate, develop, and implement individualized treatment plans for residents experiencing communication, cognitive, voice, language, and swallowing disorders. They will collaborate closely with physicians, nursing staff, rehabilitation professionals, residents, and families to promote optimal outcomes.
Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide speech, language, cognitive, communication, and swallowing therapy services
- Absess and manage dysphagia and recommend appropriate dietary modifications
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary team members to coordinate resident care
- Educate residents, families, and caregivers regarding treatment goals and progress
- Participate in care planning meetings and discharge planning as needed
- Ensure compliance with all regulatory, ethical, and facility standards
Qualifications
- Current California Speech-Language Pathologist license required
