Concord Post-Acute is seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our rehabilitation team on a per diem basis. Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide speech, language, cognitive, communication, and swallowing therapy services
• Assess and manage dysphagia and recommend appropriate dietary modifications
• Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary team members to coordinate resident care
• Educate residents, families, and caregivers regarding treatment goals and progress
• Participate in care planning meetings and discharge planning as needed
• Ensure compliance with all regulatory, ethical, and facility standards

Requirements

• Current California Speech-Language Pathologist license required
