Role: Speech Language Pathologist (SLP) in a home health setting, focused on evaluating and treating patients with speech, language, cognitive, and swallowing disorders. Responsibilities: Conduct comprehensive assessments, develop individualized care plans, provide evidence-based therapy, educate patients and caregivers, monitor progress, adjust treatments, document care, and collaborate with interdisciplinary teams to ensure coordinated, patient-centered care in the home environment. Qualifications & Skills: Master's or Doctoral degree in Speech-Language Pathology, current licensure, minimum one year of clinical experience (home health preferred), excellent communication skills, clinical judgment, ability to work independently, valid driver’s license, and CPR certification. Experience with swallowing and cognitive-communication disorders and adaptability to home environments are advantageous.
