About the role

This is a Full-Time benefited position. Position works 4 10-hour shifts, with one being remote. Position does not require weekends or holidays.

Location

Located at Scripps Green Torrey Pines Clinic, with occasional site travel as needed.

Responsibilities

  • Navigate patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient.
  • Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance.
  • Provide in person, clinic visits as needed at the initiation of care and throughout transitions of care throughout all of our hematology/oncology clinic sites, as needed.
  • Formulate custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.
  • Provide disease and treatment specific education tailored to meet patient’s individual goals of care.
  • Onsite rounding throughout treatment plan implementation as needed to provide needed in person patient evaluation.
  • Provide direct, hands-on nursing care, as needed.
  • Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Assist patients with social services needs by identifying resources in the community and referring to these programs.
  • Collaborate with system wide authorizations team, scheduling in other departments, as needed to ensure patient receives timely and comprehensive care.

Requirements

  • BSN plus 3 years’ recent clinical nursing experience in a variety of clinical settings.
  • CA RN License.
  • BLS if working in a clinical setting.
  • ONS Chemotherapy and Biotherapy certification.

Preferred

  • MSN.
  • OCN if working in Hematology, ONN-CG if working in Oncology area.
  • Ambulatory experience preferred.
  • Minimum of two years of outpatient experience managing the care of patients in oncology or a related specialty.

Please also note this range is applicable for employees who reside in California only. A geographical pay differential may be applied for remote employees who reside out of state.
