RN Specialty Nurse Navigator - GI/Oncology - Torrey Pines Clinic

About the role

This is a full-time benefited position working 4 ten-hour shifts, with one remote day. The position does not require weekends or holidays. Located at Scripps Green Torrey Pines Clinic, with occasional site travel as needed.

Responsibilities

• Navigate patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient.
• Establish specific, measurable, attainable, relevant, and time-specific nursing goals to guide the care planning process and promote self-reliance.
• Provide in-person clinic visits as needed at the initiation of care and throughout transitions of care across all hematology/oncology clinic sites.
• Formulate custom care plans from assessment and clinical information demonstrating the application of clinical guidelines, including standards of care.
• Provide disease and treatment-specific education tailored to meet patients' individual goals of care.
• Conduct onsite rounding throughout treatment plan implementation as needed to provide in-person patient evaluation.
• Provide direct, hands-on nursing care as needed.
• Assist patients in establishing, modifying, or reinforcing a social support network to promote self-reliance.
• Assist patients with social services needs by identifying resources in the community and referring to these programs.
• Collaborate with the system-wide authorizations team and scheduling in other departments to ensure patients receive timely and comprehensive care.

Requirements

• BSN plus 3 years’ recent clinical nursing experience in a variety of clinical settings.
• CA RN License.
• BLS certification if working in a clinical setting.
• ONS Chemotherapy and Biotherapy certification.

Preferred Qualifications

• MSN.
• OCN certification if working in Hematology.
• ONN-CG certification if working in Oncology.
• Ambulatory experience.
• Minimum of two years of outpatient experience managing the care of patients in oncology or a related specialty.

Pay

$62.80–$91.07/hour.

Four 10-hour shifts per week, including one remote day. No weekends or holidays required.
