Responsibilities

  • Navigate patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient.
  • Establish specific, measurable, attainable, relevant and time-specific nursing goals to guide the care planning process and promote self-reliance.
  • Provide in-person, clinic visits as needed at the initiation of care and throughout transitions of care throughout all of our hematology/oncology clinic sites, as needed.
  • Formulate custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.
  • Provide disease and treatment-specific education tailored to meet patient’s individual goals of care.
  • On-site rounding throughout treatment plan implementation as needed to provide needed in-person patient evaluation.
  • Provide direct, hands-on nursing care, as needed.
  • Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance.
  • Assist patients with social services needs by identifying resources in the community and referring to these programs.
  • Collaborate with the system-wide authorizations team, scheduling in other departments, as needed to ensure patient receives timely and comprehensive care.

Requirements

  • Bachelor of Science in Nursing (BSN) plus 3 years’ recent clinical nursing experience in a variety of clinical settings.
  • Current California Registered Nurse (RN) license.
  • BLS certification if working in a clinical setting.
  • ONS Chemotherapy and Biotherapy certification.

Qualifications

  • Master of Science in Nursing (MSN) preferred.
  • OCN certification if working in Hematology, ONN-CG certification if working in Oncology area.
  • Ambulatory experience preferred.
  • Minimum of two years of outpatient experience managing the care of patients in oncology or a related specialty.

Skills

  • Strong communication skills.
  • Ability to manage multiple tasks simultaneously.
  • Knowledge of oncology and hematology care.
  • Proficiency in clinical guidelines and standards of care.

Pay

Position Pay Range: $62.80-$91.07/hour

Schedule

This is a Full-Time benefited position. Position works 4 10-hour shifts, with one being remote. Position does not require weekends or holidays. Location is at Scripps Green Torrey Pines Clinic, with occasional site travel as needed.
