Nurse Navigator - Oncology Clinic

Role: The Nurse Navigator (NN) is a licensed registered nurse with disease-specific expertise who provides individualized support to patients, families, and caregivers to improve access to quality care throughout the cancer continuum. Responsibilities: Serve as a subject matter expert in navigation, enhance patient care timeliness, collaborate with program leaders, and perform comprehensive assessments using the nursing process. Coordinate multidisciplinary care, support performance improvement initiatives, and act as a preceptor or mentor. Maintain competency, stay updated with advances in cancer care, and ensure compliance with policies and standards. Qualifications & Skills: Associates or Bachelor's degree in Nursing, RN licensure, BLS certification, minimum 3 years of nursing experience, including 2 years in disease-specific or outpatient care preferred. Must demonstrate strong critical thinking, time management, and ability to respond decisively in fast-paced environments. Key Duties: Facilitate patient-centered care, bridge resources, support program growth, and promote evidence-based practices, ensuring safe, efficient, and compassionate care delivery.
