Nurse Navigator - Oncology Clinic

About the Role

The Nurse Navigator (NN) is a licensed registered nurse with disease-specific expertise, providing individualized support to patients, families, and caregivers to improve care access and adherence throughout the cancer continuum.

Responsibilities

• Serve as a subject matter expert in navigation, enhancing care timeliness and access.
• Collaborate with program leaders to assess community needs and improve navigation programs.
• Deliver patient-centered care using the nursing process, ensuring effective communication and coordination across multidisciplinary teams.
• Coordinate care at clinics and rounds, identify service gaps, and bridge resources.
• Participate in professional development, mentorship, and performance improvement activities.
• Ensure compliance with organizational policies, legal, and accreditation standards.

Requirements

• Associates or Bachelor's Nursing degree (preferred), with current RN licensure (single or multi-state).
• Minimum 3 years of nursing experience, with at least 2 years in disease-specific or outpatient care preferred.
• Certifications in BLS required.
• Strong critical thinking, time management, and emergency response skills in a fast-paced environment.
