Specialty Nurse Navigator
About the role
The Nurse Navigator will help navigate patients and families through the Cancer Service Line and other pertinent clinical services. In collaboration with the Specialty Team, coordinates the care of cancer patients including assisting with the completion of an individualized needs assessment and Pre & Post-Operative appointments.
Responsibilities
- Serves as a patient advocate.
- Assist as a facilitator for collaboration with multidisciplinary health team members.
- Focuses on patient satisfaction including timely testing and treatments.
- Maintains current knowledge and skills specializing in Oncology.
- Facilitates access of care to patients by educating and assisting patients in securing future needed appointments, communicating medical testing results.
- Maintains a communication system which ensures comprehensive and timely transmission of information to physicians, nursing staff, and the multidisciplinary team.
- Participates in continuous process improvement efforts to improve patient outcomes, assures appropriate utilization of resources and supports efforts to improve patient satisfaction.
- Respects the rights and personal dignity of the patients and recognizes that each patient is an individual with unique health care needs.
- Review, maintain, and follow-up daily on work queue.
Requirements
- Current Licensed Vocational Nurse (LVN) required
- 1-2 years of navigator experience, highly preferred
- Oncology experience, highly preferred
- Oncology certification within 1 year of hire preferred

- Demonstrates teaching and clinical abilities
- Demonstrates advanced practice expertise in specialty area
- Demonstrates nursing leadership, and the ability to expand base of nursing knowledge through support and development of nursing research
- Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team

Pay
Anticipated hourly range: $23.00 per hour - $35.03 per hour
