About the Role
The Nurse Navigator is a leadership position supporting patient care teams by coordinating care, providing education, and ensuring high-quality, safe, and timely care delivery. They manage inpatient and outpatient populations, facilitate discharge planning, and monitor patient outcomes, quality, and length of stay metrics. The role involves collaboration with healthcare professionals, mentoring staff, and promoting best practices within a patient-centered care model.
Responsibilities
- Coordinate patient care from pre-op to recovery and follow-up.
- Provide education to patients and families.
- Conduct inpatient rounding and assist with discharge planning.
- Collaborate with physicians, nurses, and support staff.
- Monitor quality metrics, patient outcomes, and documentation accuracy.
- Serve as a clinical resource and mentor to staff.
- Support program growth, physician relationships, and community outreach.
Requirements
- Graduate of an accredited nursing program with current RN license and BLS certification.
- 2–3 years of nursing experience; BSN, ACLS, and orthopedic or case management experience preferred.
- Physical ability to lift up to 50 lbs, stand, walk, and perform bending or reaching activities.
