South County Respite - Residential Counselor- Relief Per Diem Southbridge

About the role

Open Sky Community Services is an organization dedicated to providing support and services to individuals facing mental health challenges. We offer a range of programs, including Respite programs, which provide short-term group living environments for adults diagnosed with mental health and/or substance use disorders.

Responsibilities

• Assist in developing individualized, person-centered treatment plans.
• Aid individuals in accessing and/or providing transportation.
• Create opportunities for the development of valued roles and personal relationships in the community.
• Review and complete all required documentation.
• Support individuals to live as independently as possible, while providing evidence-based treatment and care.
• Act as a strong advocate for individuals served and promote community integration.

Requirements

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License and acceptable driving record, required.
• Reliability, willingness to learn, and openness to new opportunities.
