Role Overview

The Social Worker provides psychosocial assessments, supportive counseling, crisis intervention, and resource referrals to clients and families receiving services through Healthy Beginnings and Mobile Health Clinic programs.

Key Responsibilities

• Participate in case planning and facilitate care transitions
• Aid clients in obtaining financial resources and government entitlements
• Collaborate with healthcare team members and community agencies
• Document interactions, interventions, and actions in medical records
• Provide education, counseling, and advocacy to support client adaptation and goal setting
• Utilize crisis intervention and therapy as needed
• Support staff training and community outreach efforts
• Develop community relationships and tracking systems to enhance service delivery
• Perform insurance audits and assist with patient insurance enrollment

Qualifications & Skills

• Master's or Bachelor's Degree in Social Work
• Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), or Certified Advanced Social Worker (ACSW) certification preferred
• Valid driver’s license and auto insurance
• Experience in social services, crisis intervention, and working with high-risk populations
• Strong communication, collaboration, and documentation skills
