Social Worker (LSW/LCSW) - Community Health Programs

About the role

The Social Worker provides psychosocial assessments, supportive counseling, crisis intervention, and resource referrals to clients and families enrolled in Healthy Beginnings Services and Mobile Health Clinic programs. They collaborate with interdisciplinary teams to facilitate care transitions, develop intervention plans, and support high-risk populations. The role involves documenting interactions, advocating for financial and social services, and providing education and counseling to promote client well-being.

Responsibilities

• Provide psychosocial assessments, supportive counseling, crisis intervention, and resource referrals

Qualifications

• Master's or Bachelor's Degree in Social Work
• Licensure: LCSW or LSW
• Valid driver's license
• At least 1+ years of related experience
