About the role
The Social Worker provides professional psychosocial services to clients and families eligible for Healthy Beginnings and Mobile Health Clinic programs.
Responsibilities
- Conducting assessments
- Offering supportive counseling
- Crisis intervention
- Facilitating care transitions
- Connecting clients with community resources and financial assistance
- Collaborating with interdisciplinary teams to develop and implement individualized care plans
- Maintaining accurate documentation of interactions
- Promoting community engagement
- Participating in team meetings and community coalitions
Qualifications
- A Master's or Bachelor's Degree in Social Work
- Licensure (LCSW or LSW)
- At least 1+ years of relevant experience
