About the Role
The Social Worker will assist and provide supportive feedback and resources to individuals and families as they face Alzheimer’s and dementia-related issues in person, via phone, emails, and other online communication methods.
Responsibilities
- Provide intake and assessment to all clients to understand the progression of Alzheimer’s in the individual and individual needs; report findings to Center Manager and therapeutic specialist.
- Assess and address safety and/or risk and develop safety and intervention plans accordingly.
- Create and facilitate education programs with subject matter experts aimed toward caregivers.
- Provide counseling, educational information, and resource referrals to clients in person, by phone, or via online communication methods.
- Link callers and their families to appropriate services and programs.
- Conduct training for caregivers, first responders, first contacts, and professionals supporting people with Alzheimer’s.
- Facilitate support groups for caregivers in need.
- Represent AFA at external conferences, seminars, and events as needed.
- Assist with special projects and other duties as assigned within the scope of the role.
Requirements
- Master’s Degree in Social Work or related field (Psychology, Counseling, Mental Health, or Gerontology) and NY State license.
- Minimum of 2 years post-master’s experience specifically with an elder population; Alzheimer’s-related experience a plus.
- Intermediate computer proficiency in MS Office (Word, Excel, PowerPoint, Outlook).
- Excellent interpersonal, listening, verbal, and written communication skills.
- Strong organizational skills and ability to coordinate multiple tasks and responsibilities.
- Capacity to work independently and collaboratively with a team.
- Ability to work efficiently and effectively under pressure.
Preferred Qualifications
- Bilingual (English/Spanish) is a plus.
