Social Worker MSW (Per Diem) - PACE

About the role

This is a per diem/on call position. This position will see participants in south of Snohomish county to south of King county. As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System.

Responsibilities

• Spending a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues.
• Accessing community and financial resources for participants and ensuring continuity of care across the continuum.
• Conducting social work assessment and intervention in other care settings, including the participant's home, as needed.

Requirements

Required Qualifications:

• Master's Degree in Social Work
• Washington Clinical Independent Social Worker Associate License upon hire.
• Or Washington Clinical Independent Social Worker Associate Temporary License upon hire.
• Or Washington Clinical Independent Social Worker License upon hire.
• Or Washington Clinical Independent Social Worker Temporary License upon hire.
• 1 year of work experience with geriatric population
• 2 years of Social work with geriatric age groups from diverse ethnic cultures.

Qualifications

Preferred Qualifications:

• Experience working with diverse populations
• Knowledge of geriatric care and services
• Experience with community resource coordination

Skills

Skills required for the role include:

• Excellent interpersonal skills
• Strong communication and collaboration abilities
• Ability to assess and address social, emotional, cognitive, and ethnic factors impacting care
• Proficiency in using technology for case management and documentation

Pay

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Schedule

The position is a per diem/on call basis.
