Social Worker MSW (Per Diem)

About the role

This is a per diem/on-call social work position serving participants in the south of Snohomish and King counties. As part of an interdisciplinary team, you will provide direct social work services through one-on-one and group counseling, assessments, and collaboration with other team members.

Responsibilities

• Develop individualized care plans by evaluating social, emotional, cognitive, and cultural factors.
• Connect participants with community and financial resources.
• Conduct assessments and interventions in various settings, including home visits, to support older adults in maintaining independence.

Requirements

• Master's Degree in Social Work.
• Washington Clinical Social Worker License or temporary license.
• At least 1 year of geriatric experience.
• At least 2 years of experience working with diverse ethnic older populations.
