Social Worker (MSW)

About the role

We are looking for hospice medical social workers who are passionate about helping patients get the care they deserve and making a meaningful impact in others’ lives. In this role you will conduct psychosocial evaluations, provide ongoing counseling to patients and families during their end-of-life journey, and offer emotional support in accordance with the plan of care. You will join a supportive leadership team with professional development opportunities and room for advancement.

Responsibilities

• Conduct psychosocial evaluations of patients and families
• Provide ongoing counseling to patients and families throughout the end-of-life journey
• Offer emotional support to patients and families as needed, aligned with the plan of care

Qualifications

• MS degree in social work from an accredited school approved by the Council of Social Work Education
• Current state license as a social worker
• 2+ years of clinical work experience, preferably in healthcare or hospice
• Reliable transportation
• Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs while bearing the weight of an average adult
