Social Worker (MSW)

About the Role

We are looking for hospice medical social workers who are committed to creating meaningful experiences for your patients and their families. As a hospice medical social worker, you will be responsible for psychosocial evaluations, and ongoing counsel of patients and families during their end-of-life journey. Working in accordance with the plan of care, you will provide emotional support to patients and families when it's needed most.

Responsibilities

• Provide psychosocial evaluations and ongoing counsel to patients and families during their end-of-life journey
• Provide emotional support to patients and families in accordance with the plan of care

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation
• Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively
