Role Overview

The Medical Social Worker provides social services under physician guidance to support patients' emotional, social, and environmental needs related to their health.

Responsibilities

• Assist patients, families, and healthcare teams in understanding social factors impacting health and develop coping strategies.
• Contribute to creating comprehensive, individualized care plans.
• Educate team members on community resources available to patients.
• Function as a Bereavement Coordinator, managing bereavement services and developing related care plans.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Current state licensure as a Licensed Social Worker.
• CPR certification, valid driver's license, and reliable transportation.
• At least one year of social work experience in a healthcare setting.

Preferred Skills

• Experience in bereavement coordination and care planning.
