About the role
The position will see participants in North Shoreline to Central Seattle. The schedule is full-time, weekdays on day shift.
Responsibilities
- Provides social work services to participants in a manner consistent with the mission and core values of Providence Health System.
- Spend a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues.
- Accesses community and financial resources for participants and ensures continuity of care across the continuum.
- Conducts social work assessment and intervention in other care settings, including the participant's home, as needed.
Requirements
- Master's Degree in Social Work
- Washington Clinical Independent Social Worker Associate License or Temporary License upon hire
- 2 years Social work with geriatric age groups from diverse ethnic cultures
- 1 year Work experience with geriatric population
