Social Worker - PACE program

About the role

The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain independent and safe in their own homes. This role involves building lasting connections with participants, supporting their independence and mobility, collaborating with an interdisciplinary team, and ensuring participant and caregiver needs are met.

Responsibilities

• Assessment, treatment, teaching, and counseling of participants, caregivers, or other appropriate representatives
• Provision of resources and ongoing case management
• Advocacy to ensure participant and caregiver needs are met and addressed
• Disenrollment procedures
• Liaison between the Interdisciplinary Team (IDT), caregiver representatives, and community agencies

Requirements

• Master of Social Work (MSW) degree from a school of Social Work accredited by the Council on Social Work Education
• At least one year of social work experience in a healthcare setting on a multidisciplinary team (preferable)
• A current Florida license, or registration with the State of Florida as a Registered Intern actively working on licensure, or being in the process of becoming a Registered Intern with the State of Florida and having a registration within 90 days of hire
• Medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact
• Reliable transportation for field-based positions

Qualifications

• Must have a valid driver’s license and reliable transportation
• Must provide proof of valid automobile insurance

• Ability to work effectively in a multidisciplinary team environment
• Excellent organizational and time management skills
• Knowledge of PACE program principles and best practices

Schedule

Hours may vary depending on the needs of the program and may require evening and weekend work.
