Social Worker-PACE (Murrieta)

About the role

Neighborhood Healthcare PACE is a managed medical plan serving over 650 senior participants through two centers in Riverside County. Our mission is to ensure seniors remain healthy and happy at home, supported by a multidisciplinary team.

Responsibilities

• Works with the PACE Interdisciplinary Team to conduct assessments and updates.
• Coordinates family conferences and provides counseling services.
• Screens and develops share of cost for out-of-home placements.
• Communicates with healthcare providers and external agencies.
• Maintains documentation and supports discharge planning.
• Provides redirection and support for behavioral needs.
• Acts as an advocate and liaison for participants.

Requirements

• Master’s degree in social work required.
• One year of experience working with the elderly population required.
• Basic Life Support (BLS) certification required upon hire.
• Bilingual (English/Spanish) preferred.

Qualifications

• One year of social work experience preferred.
• Experience working in a community-based setting or geriatric program preferred.
• Excellent verbal and written communication skills.
• Knowledgeable about counseling theories and techniques.
• Knowledgeable about community resources.
• Ability to establish and maintain good interpersonal relationships.
• Excellent planning and organizational ability.
• Ability to work as part of a team and independently.
• Ability to work with confidential information professionally.

Physical Requirements

• Ability to lift/carry 10 lbs.
• Ability to stand for long periods of time.

Pay

$80,200 - $115,500 annually, depending on experience.
