Social Worker MSW - PACE

About the role

As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System. Spends a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues. Based on comprehensive assessments, provides perspective on the social, emotional, cognitive and ethnic factors that impact the creation of an individualized integrated plan of care. Accesses community and financial resources for participants and ensures continuity of care across the continuum. Conducts social work assessment and intervention in other care settings, including the participant's home, as needed.

Responsibilities

• Provides one-on-one and group counseling to participants.
• Collaborates with interdisciplinary team members to address participant needs.
• Conducts comprehensive social work assessments to inform individualized care plans.
• Accesses community and financial resources to support participant needs.
• Ensures continuity of care across care settings, including home visits when necessary.

Requirements

• Master's Degree in Social Work.
• Upon hire: Washington Clinical Independent Social Worker Associate License or Washington Clinical Independent Social Worker Associate Temporary License or Washington Clinical Independent Social Worker License or Washington Clinical Independent Social Worker Temporary License.
• 2 years of social work experience with geriatric age groups from diverse ethnic cultures.
• 1 year of work experience with the geriatric population.

Pay

The base pay range for this position is $38.56 – $59.88 per hour. Additional compensation may be available, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

$7,500 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.

Monday–Friday, Day shift.
