Role Overview
This position involves providing comprehensive biopsychosocial/spiritual assessments, diagnoses, interventions, and advocacy for patients and their support systems. It includes collaborating with interdisciplinary teams to develop and implement evidence-based, person-centered care plans that respect patient self-determination, reduce disparities, and adhere to professional ethical standards.
Responsibilities
- Conduct biopsychosocial/spiritual assessments and provide appropriate treatment or interventions.
- Collaborate with patients and support systems to create personalized care plans.
- Work closely with multidisciplinary teams across inpatient and outpatient settings.
- Support patient advocacy, address health disparities, and promote equitable access to care.
Qualifications & Skills
- Master's Degree in Social Work (MSW) or equivalent.
- Licensure: Oregon Clinical Social Work Associate License (within 60 days) for MSW; Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License for LCSW.
- Minimum two years of relevant social work experience, preferably in healthcare, hospice, or bereavement.
- Knowledge of neurobiology, behavioral change, and lifespan development theories.
- Ability to work independently and collaboratively, demonstrating cultural sensitivity and ethical practice.
