About the role
The on-call Social Worker provides biopsychosocial/spiritual assessments, diagnoses, treatment, advocacy, and develops person-centered care plans in collaboration with patients and multidisciplinary teams. This role involves working closely with diverse patient populations across inpatient and outpatient settings, supporting the organizational mission and values.
Responsibilities

Requirements
For MSW candidates:
- Master’s Degree in Social Work
- Oregon Clinical Social Work Associate License within 60 days of hire
- Preferred: two years of experience in healthcare, hospice, or bereavement settings
For LCSW candidates:
- Master’s Degree in Social Work from an accredited school
- Licensure as an LCSW in Oregon or Washington
- Two years of relevant experience, with preference for care management in acute care
