Social Worker MSW / LCSW

About the Role

This role involves providing comprehensive biopsychosocial/spiritual assessments, diagnosis, treatment, and advocacy for patients and their support systems.

For CSWA: Under supervision, supports evidence-informed, person-centered care plans in collaboration with the interdisciplinary team, respecting patient self-determination and adhering to NASW standards.

For LCSW: Independently conducts assessments, diagnoses, and treatment interventions, working across inpatient and outpatient settings to reduce health disparities and promote patient-centered care.

Responsibilities

• Conduct biopsychosocial/spiritual assessments, diagnosis, and treatment.
• Develop and implement care plans in collaboration with patients and interdisciplinary teams.
• Work closely with various departments to support diverse patient populations.
• Respect patient autonomy, promote health equity, and adhere to professional ethics and standards.

Qualifications

• MSW from an accredited program.
• For CSWA: Oregon Clinical Social Work Associate License within 60 days of hire.
• For LCSW: Relevant licenses for independent practice.
• Minimum two years of clinical social work experience.
• Preferred experience in healthcare, hospice, or acute care.
