About the Role
This position involves providing comprehensive biopsychosocial and spiritual assessments, diagnoses, and interventions for diverse patient populations. Responsibilities
- Conduct comprehensive biopsychosocial and spiritual assessments for diverse patient populations.
- Develop and implement evidence-informed, person-centered care plans independently (LCSW) or under supervision (Social Worker MSW).
- Collaborate with interdisciplinary teams to ensure integrated, ethical, and culturally competent care.
- Advocate for patients and reduce barriers to healthcare access.
- Coordinate care to support patient self-determination and improve health outcomes.
- Adhere to professional standards and ethical guidelines in all interventions.
Requirements
- Master’s degree in Social Work (MSW) from a CSWE-accredited program.
- Appropriate licensure (e.g., LCSW) for independent practice; supervised licensure acceptable for Social Worker MSW role.
- Experience in healthcare settings, preferably with diverse or vulnerable populations.
- Demonstrated cultural competence and commitment to reducing health disparities.
- Strong collaboration and communication skills.
