About the Role

This position involves providing comprehensive biopsychosocial and spiritual assessments, diagnoses, and interventions for diverse patient populations. You will independently develop and implement evidence-informed, person-centered care plans in collaboration with interdisciplinary teams, ensuring adherence to ethical standards and reducing health disparities. The role emphasizes patient self-determination, care coordination, and reducing barriers to access in a supportive, inclusive work environment committed to serving vulnerable communities.

Responsibilities

• Conduct comprehensive biopsychosocial and spiritual assessments for diverse patient populations.
• Develop and implement evidence-informed, person-centered care plans independently (LCSW) or under supervision (Social Worker MSW).
• Collaborate with interdisciplinary teams to ensure integrated, ethical, and culturally competent care.
• Advocate for patients and reduce barriers to healthcare access.
• Coordinate care to support patient self-determination and improve health outcomes.
• Adhere to professional standards and ethical guidelines in all interventions.

Requirements

• Master’s degree in Social Work (MSW) from a CSWE-accredited program.
• Appropriate licensure (e.g., LCSW) for independent practice; supervised licensure acceptable for Social Worker MSW role.
• Experience in healthcare settings, preferably with diverse or vulnerable populations.
• Demonstrated cultural competence and commitment to reducing health disparities.
• Strong collaboration and communication skills.
