This role involves providing comprehensive biopsychosocial/spiritual assessments, diagnoses, and interventions for diverse inpatient and outpatient populations.

Responsibilities

• Independently develop and implement evidence-based, person-centered care plans in collaboration with multidisciplinary teams.
• Ensure adherence to ethical standards and work to reduce health disparities.
• Perform biopsychosocial/spiritual assessments and support patient self-determination (CSWA under supervision).

Requirements

• Master’s Degree in Social Work.
• Relevant licensure (LCSW or CSWA).
• Experience in healthcare or acute care settings.
