Social Worker - MSW

About the role

Provides psychosocial services to patients, including those on in-center and home dialysis, utilizing social work theories and best practices. Works collaboratively with the healthcare team to promote positive adjustment, rehabilitation, and enhance quality of life. Educates and supports staff on the emotional and behavioral impacts of Chronic Kidney Disease, ensuring comprehensive patient care.

Responsibilities

• Conducts patient assessments, develops care plans, and offers counseling and education tailored to individual needs.
• Facilitates transplant referrals, addresses barriers to treatment adherence, and connects patients with community resources.
• Supports patients with advance directives, insurance issues, and financial assistance.
• Participates in quality improvement activities, team meetings, and staff training.
• Ensures documentation aligns with policies and regulatory standards.

Requirements

• Master's in Social Work with a valid state license.
• 2-5 years of relevant experience.
• Strong skills in patient assessment, counseling, education, and collaboration with multidisciplinary teams.
• Ability to handle psychosocial challenges, maintain documentation, and support quality initiatives.
• Physical demands typical of healthcare settings; some travel may be required if multiple facilities are involved.
• No supervision is provided.

Pay

$54,000 to $91,000 annually.
