Social Worker - MSW
About the Role
Provides psychosocial services to dialysis patients, supporting their emotional, psychological, and behavioral needs to promote positive adjustment, rehabilitation, and improved quality of life. Works collaboratively with the healthcare team to assess patient needs, develop care plans, and deliver counseling and education tailored to individual learning styles and barriers. Assists patients with transplant referrals, community resource access, and understanding of advance directives. Addresses treatment adherence, quality outcomes, and patient satisfaction through psychosocial interventions and participation in quality improvement activities. Supports patient admission, insurance coordination, and staff training. Maintains documentation and adheres to policies, working within licensed scope of practice.
Responsibilities
- Conduct psychosocial assessments of dialysis patients.
- Develop and implement individualized care plans.
- Provide counseling to support emotional, psychological, and behavioral needs.
- Support treatment adherence and educate patients on disease management.
- Facilitate transplant referrals and access to community resources.
- Assist patients with insurance coordination and understanding of advance directives.
- Contribute to quality improvement activities and patient satisfaction initiatives.
- Support staff training and interdisciplinary collaboration.
- Maintain accurate documentation and comply with organizational policies.
Requirements
- Master’s degree in Social Work.
- Valid state social work license.
- 0–2 years of related experience.

Pay
Annual salary range of $53,000 to $89,000.
