Social Worker - MSW

Seeking a Social Worker to provide psychosocial services to patients with chronic kidney disease, including in-center and home dialysis. Responsibilities Include Conducting patient assessments, care planning, and providing counseling to address psychosocial needs and support treatment adherence.Collaborating with the healthcare team to develop and implement personalized interventions aimed at improving quality of life and treatment outcomes.Offering supportive counseling, education on treatment options, transplant referral assistance, and facilitating access to community resources.Supporting patients with advance directives, end-of-life decisions, and addressing barriers to treatment adherence.Participating in quality improvement activities, reporting on quality indicators, and ensuring compliance with policies and documentation standards.Assisting with patient admission, discharge, insurance coordination, and providing staff training on psychosocial topics. Qualifications Include Master’s degree in Social Work and valid state license.2-5 years of relevant experience. Skills Required Knowledge of social work theory and methods, patient education, and community resource navigation.Strong communication, counseling, and interdisciplinary collaboration skills.Ability to assess psychosocial status and develop effective care plans. Work environment: No supervision required; physical demands include potential travel between facilities.
