Social Worker - MSW

About the role

Provides psychosocial services to in-center and home dialysis patients, collaborating with healthcare teams to promote adjustment, rehabilitation, and quality of life.

Responsibilities

• Assessing patients' psychosocial status, strengths, and needs to support treatment and rehabilitation goals.
• Developing care plans with patients and the team, using social work methods and education programs.
• Offering counseling, education, and support for dialysis adjustment, transplant processes, and advance directives.
• Facilitating referrals to community resources and vocational rehabilitation.
• Supporting patient adherence, quality outcomes, and participation in quality improvement activities.
• Providing patient education on treatment options, rights, and responsibilities.
• Assisting with patient admission, addressing immediate concerns, and supporting continuity of care.
• Collaborating with insurance and financial teams to address coverage and payment issues.
• Training staff on psychosocial topics and participating in team meetings.

Qualifications

• Master's in Social Work and state licensure.
• 2–5 years of related experience.

• Strong assessment, counseling, education, and collaboration skills, with knowledge of healthcare and social work practices.
