About the role
Provides psychosocial services to in-center and home dialysis patients, collaborating with healthcare teams to promote adjustment, rehabilitation, and quality of life.
Responsibilities
- Assessing patients' psychosocial status, strengths, and needs to support treatment and rehabilitation goals.
- Developing care plans with patients and the team, using social work methods and education programs.
- Offering counseling, education, and support for dialysis adjustment, transplant processes, and advance directives.
- Facilitating referrals to community resources and vocational rehabilitation.
- Supporting patient adherence, quality outcomes, and participation in quality improvement activities.
- Providing patient education on treatment options, rights, and responsibilities.
- Assisting with patient admission, addressing immediate concerns, and supporting continuity of care.
- Collaborating with insurance and financial teams to address coverage and payment issues.
- Training staff on psychosocial topics and participating in team meetings.
Qualifications
- Master's in Social Work and state licensure.
- 2–5 years of related experience.
Skills
- Strong assessment, counseling, education, and collaboration skills, with knowledge of healthcare and social work practices.
