About the role

The Valley Health System, comprising Valley Hospital and other facilities, provides comprehensive healthcare services to Southern Nevada residents and visitors. Founded in 1972, Valley Hospital is part of the Valley Health System, which includes acute care hospitals, physician practices, and outpatient services. The system focuses on delivering exemplary patient care and training future healthcare providers.

Responsibilities

• Responsible for the appropriate and timely discharge and transfer arrangements for patients referred to the Care Management Department.

Qualifications

• Education: Master’s degree
• Experience: At least one year of actual work experience in a hospital setting or an ongoing plan for consultation between the social worker and a qualified social worker must be developed.
• Knowledge: Knowledge of social and physical factors that affect functional status at discharge and knowledge of community resources to meet post-discharge clinical and social needs.
• Licenses/Certifications: Currently licensed by the Nevada State Board of Examiners for Social Workers (LMSW).
• Skills: Ability to demonstrate the knowledge and skills necessary to provide care/service appropriate to the age of the patients served on the assigned unit/department.
