Job Summary

At Brighton/Suncrest Hospice, we aim to enhance hospice care in your area through increased staff-to-patient ratios, leading to more frequent clinical visits and reduced clinician caseloads. As a CHAP-certified hospice, we seek candidates committed to delivering top-tier care to patients and their families.

What You'll Be Doing

• Assesses psychosocial status of clients and social/emotional factors related to their illness, needs for care, and responses to treatment.
• Provides direct social casework services, including goal setting, counseling, and planning, and implementing interventions.
• Counsels individuals and families, advises on available community resources, and updates case records.
• Participates in developing and updating the client's care plan, observes and reports client progress to physicians, and attends relevant meetings.
• Maintains accurate and timely client care records, communicates with other healthcare professionals, and establishes effective working relationships.

Requirements

• Master’s degree in Social Work
• Thorough knowledge of social work principles and methods
• Knowledge of individuals and groups with various social, economic, emotional, or health issues
• Ability to exercise independent professional judgment
• Effective communication skills (both oral and written)
