Social Worker - MSW

Provides psychosocial services to dialysis patients, supporting their emotional, psychological, and behavioral well-being. Works collaboratively with the healthcare team to assess patient needs, develop care plans, and deliver counseling aimed at improving treatment adherence, quality of life, and adjustment to dialysis or transplant processes. Conducts patient education on kidney disease, treatment options, and support services, ensuring understanding of patient rights and facilitating referrals to community resources. Assists with discharge planning, insurance coordination, and addressing barriers to treatment adherence, while supporting quality improvement initiatives and participating in team meetings. Qualifications include a Master's in Social Work, a state-required license, and 2–5 years of relevant experience. The role involves patient assessment, counseling, education, and collaboration with interdisciplinary teams.
