Responsibilities

• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
• Must have advanced training in hospital or long-term administration.
• Be knowledgeable of reimbursement regulations and nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long-term care administration.
• Be familiar with the laws, regulations, and guidelines governing personnel administration.
• Have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents, staff, and visitors based on whatever maturity level at which they are currently functioning.
• Communicate policies, procedures, regulations, reports, etc., to personnel, residents, family members, visitors, and government agencies/personnel.
• Knowledge of charting via Minimum Data Set (MDS).
• Experience completing psychosocial evaluations, social histories, cognitive, mood and behavioral assessments.
• Quarterly Care Plan meeting attendance is required and completion of treatment plans within prescribed time frames.
• Experience providing services to the elderly population and their families.
• Familiarity of Resident Rights and Advance Directives.
• Knowledge of Discharge Planning policy and procedures.
• Act as liaison and referral with psychiatric and psychological service providers as well as ancillary service providers.
• Must be a team player with strong clinical skills.
• Experience working with the elderly.

Requirements

Master’s Degree, MSW required.

Must possess a working knowledge of long-term care operational standards set forth in the Federal Register, Requirements of participation.

Experience working with the elderly.

Skills

Experience completing psychosocial evaluations, social histories, cognitive, mood and behavioral assessments.

Quarterly Care Plan meeting attendance is required and completion of treatment plans within prescribed time frames.

Experience providing services to the elderly population and their families.

Familiarity of Resident Rights and Advance Directives.

Knowledge of Discharge Planning policy and procedures.

Act as liaison and referral with psychiatric and psychological service providers as well as ancillary service providers.
