Job Summary
This position provides assessment, counseling, information and referral, advocacy services, and other resource assistance to patients directly or through consultation. Services are provided every Saturday/Sunday from 8:00 AM - 5:00 PM in the Emergency Department and 3 flexible days per week.
Responsibilities
- Completes psychosocial assessments of patients and families.
- Counsels individuals and families to help solve social and emotional problems.
- Coordinates community resources to address identified needs.
- Aids families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers, and linkage to resources outside the community.
- Provides consultation, assessment, referral, and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
- Organizes support groups.
- Documents objective, concise, and pertinent clinical data in the medical record.
- Performs other duties as assigned.
Qualifications
- Masters Degree in Social Work (MSW) required.
- Social work experience in an acute, home care, or ambulatory setting preferred.
- Strong organizational skills, verbal and written communications skills.
- Sound decision making and judgment.
- Time management skills.
- Negotiation skills.
- Proven ability to deal with conflict resolution.
- Comfortable with basic computer skills.
