Social Worker - MSW

Job Summary

This position provides assessment, counseling, information and referral, advocacy services, and other resource assistance to patients directly or through consultation. Services are provided every Saturday/Sunday from 8:00 AM - 5:00 PM in the Emergency Department and 3 flexible days per week.

Responsibilities

• Completes psychosocial assessments of patients and families.
• Counsels individuals and families to help solve social and emotional problems.
• Coordinates community resources to address identified needs.
• Aids families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers, and linkage to resources outside the community.
• Provides consultation, assessment, referral, and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
• Organizes support groups.
• Documents objective, concise, and pertinent clinical data in the medical record.
• Performs other duties as assigned.

Qualifications

• Masters Degree in Social Work (MSW) required.
• Social work experience in an acute, home care, or ambulatory setting preferred.
• Strong organizational skills, verbal and written communications skills.
• Sound decision making and judgment.
• Time management skills.
• Negotiation skills.
• Proven ability to deal with conflict resolution.
• Comfortable with basic computer skills.
