Role Overview
The Hospice Social Worker assesses the psychosocial needs of patients and families, providing resource support, short-term counseling, and crisis intervention in collaboration with interdisciplinary teams.
Responsibilities
- Conducting assessments
- Developing bereavement care plans
- Interpreting cultural and spiritual beliefs
- Documenting patient progress
- Facilitating patient decision-making
- Supporting end-of-life care
- Assisting with advanced directives, transportation, and resource referrals such as financial aid and elder abuse reports
Qualifications
- Master's Degree in Social Work
- Licensed as an LSW or LCSW
- Valid driver's license with auto insurance
- Minimum of one year healthcare social work experience
Skills & Key Duties
- Conduct psychosocial assessments and develop tailored care plans
- Provide emotional, social, and environmental support to patients and families
- Document assessments, interventions, and care updates accurately and timely
- Educate families on care options, cultural considerations, and advance directives
- Identify and respond to signs of imminent death, ensuring compassionate end-of-life support
- Utilize community resources to maximize patient benefits and coping skills
- Collaborate with interdisciplinary teams to ensure comprehensive care
The role requires sensitivity to cultural and spiritual needs, a palliative care philosophy, and the ability to adapt interventions to individual circumstances.
