Responsibilities

• Developing and implementing care plans
• Supporting family coping
• Facilitating communication about life care decisions
• Coordinating with interdisciplinary teams

• Assisting with advanced directives

Qualifications

• Master's Degree in Social Work
• Licensure (LSW or LCSW)
• Valid driver's license
• Auto insurance
• At least one year of healthcare social work experience
