Social Worker (LMSW)- Hospice

Responsibilities

• Provide emotional and logistical support to patients and their loved ones during their final days.
• Affirm patients' wishes regarding finances, funeral arrangements, and other matters.
• Navigate the grieving process for patients and their families.
• Grant end-of-life wishes and listen to patients' stories at their bedside.
• Care for hospice patients in their homes or assisted living/skilled nursing facilities.
• Meet with interdisciplinary team members once a week to discuss patient care.

Requirements

• Thorough knowledge of community resources or the ability to acquire such knowledge.
• Knowledge of key psychosocial issues related to terminally ill patients and their families.
• Excellent listening and communication skills.
• Ability to be an effective patient and family advocate.
• Commitment, compassion, and a can-do attitude.

Qualifications

• Masters of Social Work from a Council on Social Work Education-accredited school, or equivalent experience where allowable by state regulations.
• At least one year of social work experience in a healthcare setting.
• Experience working with terminally ill patients and their families.
• Empathy and compassion in dealing with seriously ill patients and their families.

Skills

• Effective communication and counseling skills.
• Ability to work well in an interdisciplinary team.
• Strong emotional support and crisis intervention abilities.
• Broad knowledge of psychosocial issues and interventions.
