Job Summary

Hospice Social Workers at Interim HealthCare are dedicated to providing exceptional, patient-centered end-of-life care. The role involves conducting psycho-social assessments, offering counseling, addressing financial and environmental concerns, and collaborating with the interdisciplinary team.

Responsibilities

  • Work under the direction of the Medical Director
  • Conduct initial and ongoing psycho-social assessments within five (5) calendar days of patient admission and update assessments as needed
  • Provide counseling to patients and families to address mental and emotional challenges
  • Identify cultural or religious needs and integrate them into the interdisciplinary care plan
  • Address anticipatory grief and refer to appropriate resources when necessary
  • Evaluate and address financial or environmental concerns, connecting families to relevant community resources
  • Provide crisis intervention and offer psycho-social support to the patient/family and the interdisciplinary team (IDG)
  • Update facility staff on psycho-social progress, especially in short-term inpatient or respite care settings
  • Contribute to the development and review of the IDG care plan, attending scheduled meetings
  • Aid in advance directives, legal, and financial matters, including power of attorney and funeral arrangements
  • Collaborate with the care team, ensuring all members understand the emotional and social aspects of end-of-life care
  • Participate in discharge planning and revise care plans as the patient’s status changes
  • Maintain accurate, timely documentation in the electronic medical record and consistently meet documentation standards

Requirements

  • Master’s Degree in Social Work from an accredited institution
  • Current Licensed Master Social Worker (LMSW) certification
  • Prior experience in hospice, palliative care, or a related field preferred
  • Excellent communication and interpersonal skills
  • Ability to work both independently and as part of a team
  • Caregiving and empathetic nature in supporting terminally ill patients and families
