Hospice Social Worker (LCSW & LiCSW)

About the role

The Social Worker will be responsible for assessing emotional factors related to terminal illness, providing emotional support, short-term crisis intervention and individual and/or family counseling when indicated, maintaining contact with appropriate community agencies, evaluating patient/family response to psychosocial interventions, assessing special needs related to cultural diversity, performing comprehensive initial psychosocial assessments, assessing the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect, and more.

Responsibilities

• Affirms emotional factors related to terminal illness
• Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated
• Maintains contact with appropriate community agencies and services
• Promotes interagency cooperation and facilitates related referrals
• Evaluates patient/family response to psychosocial interventions
• Affirms special needs related to cultural diversity including communication, space, role of family members and special traditions
• Performs comprehensive initial psychosocial assessments
• Affirms the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect

Requirements

• Minimum 1 year of Medical Social Work experience, Hospice preferred
• Licensed Clinical Social Worker (LCSW) certificate
• Understanding of hospice philosophy related to families in crisis and end of life issues
• Ability to multitask & prioritize work load
• Willingness to travel
• Strong verbal and written communication skills
