About the role
The Hospice Social Worker assesses the psychosocial needs of patients and families, providing support through resource identification, short-term counseling, and crisis intervention. Collaborating with interdisciplinary teams, they develop and implement individualized care plans to enhance family strength, maximize benefits, and support cultural and spiritual needs. Responsibilities include conducting assessments, documenting care and interventions, facilitating communication about care decisions, and addressing end-of-life needs, including advance directives and bereavement plans. The role involves making referrals for home assistance, transportation, and community resources, ensuring patient dignity and coping skills are prioritized.
Qualifications
- Master’s Degree in Social Work (Required)
- Licensed Clinical or Licensed Social Worker (LCSW or LSW)
- Minimum of 1 year healthcare social work experience in settings like hospice, hospital, or long-term care
- Valid driver’s license and auto insurance
- Strong assessment, communication, and documentation skills
- Understanding of hospice benefits, cultural sensitivity, and end-of-life care principles
